We want you to have access to our policies and to know what your responsibilities are in caring for your own health. This section of our web site is devoted to letting you know what our basic policies are and what we expect you to be responsible for as your part of your health care.

Patient Information
This is the basic information we want you, as a new patient to know about our clinic. It will tell you about our hours and appointments, telephone use, our fees, billing, insurance and collections policies. How we will handle your medical records and how you can help us to serve you. Please feel free to print this section and keep it handy for your reference.

How and When to Phone the Doctor
Everyone at one time or another has a need to call the doctor. This handy brochure, which you can print out, will give you all the basic information you should know when you call the doctor. Please pay attention to it! If you follow all the included points, it will make your contact with the doctor and his staff the most informative and productive it can be.

Required Forms
Here you will find all the forms we require of new patients. New patients should complete the New Patient Registration Form (PDF* 281KB) for every individual that is to be seen. Adults should also complete the Medical History Record Form ( PDF* 920K). Individuals under 15 should have the Pediatric History Form (PDF* 314KB) completed. You can download each of these forms for completion and bring them with you to the office, OR you can complete them online and email them to us prior to your visit. An Allergy History Form can also be found here for those patients who are to be allergy tested in our facility. Instructions for preparation for allergy testing are included. As a new patient, please keep in mind that you must bring your current medical insurance card with you each time you visit our clinic.

Payment Policy
It is important for you to understand our payment policies. You will find them here. We have included an example of our standard statement with an explanation of the various parts thereof so that you can better understand the statements you will receive. Please know that we expect you to pay what you owe, apart from your insurance company, in a timely fashion and we will hold you responsible for such!

Insurance
Most people have insurance that is provided by their employer. It is important that you fully understand the benefits of the insurance that has been provided for you by your employer. It is not the responsibility of Family Medical Care to know the details of your particular health plan. We will not alter a claim form fraudulently! Check here to learn more about your health insurance responsibilities and to review the current health insurance plans and provider networks we participate in. If you are a Medicare patient, please review the information for Medicare patients, before making an appointment with us.

Hospitals
Dr. Koenig is on the active medical staff of both St. Edward Mercy Medical Center and Sparks Regional Medical Center. He considers both of the facilities to be excellent and is happy to answer your questions about either hospital.
